Typically one doesn't drink the water in locations that do not have sanitary sewer systems...
While it is true that you cannot have "natural" water that does not have fecal coliform, due to animals and fish, the amount of it is what makes the problems...
Also the transport. If the water pipes are old, inferior quality, improperly connected and such, the sewage can leech into the water pipes... Quite common in Mexico City as they city was built on a lake and slowly sinks...
Native people often build up a tolerance to water with higher fecal coliform, so they don't get sick. That doesn't mean that you can't or won't. I have no desire to try and build up my tolerance to drinking potentially contaminated water, YMMV...
