You are not required to have a supplemental to cover the 20% co-pay and deductibles. There is on penalty if you don't get one.
Part D, on the other hand, will charge you a penalty if you don't enroll when you become 65 - they want everyone in as soon as possible, even when you don't need the coverage. The best way is to look at what you are currently taking and then do some comparisons between Walgreen/Costco/Wal-Mart/CVS/ExpressScripts, etc., with their Part D programs. And you can change each year - just remember that the Part D has a donut hole (though it is getting smaller each year) where you are responsible for all of the costs before the insurance kicks back in again.
