I use a small chain hoist which is hung inside my front compartment of the fiver, and the chain goes down through a hole I cut so that I can hoist the spare up against the bottom in between the front landing gear(I have the polar package).
There's just enough room between the truck and camper for me to open the front compartment and lower/raise the tire. It's easy to put the flat in the same place as it doesn't have to be right under the hole, the hoist and chain will pull it over and under and up. Have had it like this for several years with no problems.
