If you have no plans to ever use the gooseneck, you have 1 4" hole in your bed. If you do want to have the option to use the GN, then you have 4 more 1/2" holes in the bed for the safety chain connectors. These are 2 spring-loaded u-bolts that go through the bed and into the base and pop up when you need them. They're positioned in one of the rib valleys on each side, and only stick up about 1/8" above the ribs when not in use. That's it; 5 holes total. If you do a DIY install (I've done 2), the single hardest thing is getting up the nerve to actually cut a 4" hole in the bed of your truck. :-)
