It's the pivot point you have to worry about.
There are legitimate 5th wheel hitches that attach to the ball and the pivot point is at the same place as the typical 5th wheel hitches. Those should be fine.
There are gooseneck adapters where the pivot point is at the ball. Those create substantially more stress on the trailer frame. If the manufacturer designed for it, it's not a problem but many don't. Check with the manufacturer
