Old Biscuit, My observation and thus question would be, why did it pull off of the ball? Was it not properly torqued? Is this something that just happened due to the stresses involved? Personally I do not know how they attach it to the ball in the floor. Maybe that needs to be looked into by Anderson? Just some of my thoughts.

It doesn't really look like it pulled off the ball in the floor. One corner pulled up and it bent very badly but it looks like it is still connected to the ball.

I don't think it =did= pull off the ball, though the vertical tube may be somewhat distorted. The cross-pin is in place and doesn't look bent.

Floor mount is a gooseneck ball square shaft that slips into frame bracket bolted to truck.
Anderson hitch slips down onto that gooseball and is then secured to it (Only attachment point...the hitch frame just sits on truck bed)
Appears that the truck gooseneck is intact/secure
Personally I would suspect Anderson hitch frame bent backwards from high stress/torque load and then the tubing collapsed from the pin weight as hitch reared up & back
Took a LOT of strain, torque and DYNAMIC Loading to cause it.
Wonder what driver 'encountered'....there was mention that driver felt a 'TUG' just prior :H
