Not sure if this has been posted. Rather disconcerting. Andersen failure

They may never find out what happened here but it's an eye opener. I use the same set up so I intend to inspect it frequently for signs of failure. I don't think it's a characteristic of it or you'd hear of more incidents of similar problems.
