As to "why" this happened, all it would take is a small depression in an otherwise "flat" ground surface to allow a trailer this heavy to settle on it's suspension on one side a bit more than the other, allowing the trailer to tilt vertically just enough to chew up any clearance there may have otherwise been between the truck and trailer. Unfortunate to be sure but as you say this should remind others towing with a short bed that a slider should be considered mandatory, not optional.
