Pay attention to the instructions that came with your hitch! In detail, never assume, and do the pull test. You mentioned you had problems twice? As mentioned have the hitch inspected by a professional to make sure there are no problems with it. Should not have to be rocket science, but you must follow instructions to the letter so nothing is skipped. Good luck.
