I used that model hitch for two years without issue. It is VERY basic in operation with positive operation. There is a single wrap around jaw that is either engaged or not. It may not be the best hitch out there, but I never doubted it was dependable. I would always look at the pin to check that the jaw was around it. I always follow up with a pull test. My guess would be it was high hitched.
