Hopefully Ken will reply. My take on his post, he backed in with jaws closed, handle still closed. He forgot to open handle before backing in. He could not have looked to see if jaws were around pin, before putting safety pin in, and doing pull test.
Everyone with a B&W knows, if the jaws are closed around the pin, and the handle closed/pinned, the kingpin cannot come out.
I use the look at the jaws twice method, before moving.
B&W instructions say pull test, which saved Ken, due to his not looking close enough, to verify jaws wrapped around pin.
