Purchase a transporter tag / plate from your local DMV office. Show up at U.S. Customs with transporter plate on trailer. Have the CBP form #6059B completed and declare your trailer to the receiving agent.

Should not be a problem to import on a personal declaration as long as you accompany the item being imported. If you have a commercial entity bring the trailer across the border for you, and you are NOT present, the trailer will need to be entered (formal entry) by a broker.
