looking on the net, this appears to be a known recurring issue with this hitch, with the slide bar latch. the only 'fix' is to weld some metal back into the space where the wear has occurred. (which in my opinion is not a good solution for not a good design).
most folks that had the problem have switched to a different hitch, with dual jaws.. (Reese Signature?).
