I think the vast majority of hitch owners understand a 5th wheel hitch is a mechanical devise that the jaws don't open once their "properly closed around the pin". Some cheerleader types being the exception on every dropped the trailer thread lately.
There are several ways that a trailer can land on the trucks bed that I've seen due to;
#1. operator error #2. a actual mechanical malfunction #3. vandalism.
Reading the different trailering forums (rv and non rv) shows even more examples. GN/bumper/pintle/5th wheel hitched trailers have all found the ground or just the trucks bed.
The OP situation comes under operator error as the vast majority of dropped trailer threads goes.
Agree and agree- I have a B&W hitch and can see how due to operator error it could be dropped.
Sadly, just last week- I hitched up and then did a pull test immediately, skipping the step where I close the latch arm. Fortunately for me- the landing gear were still down on the ground AND the hitch held the pin (pressure on the jaws were enough).
But, mental mistakes happen. It can happen to anyone and any brand. We are human, therefore we aren't perfect.
