Most likely operator error, and not a failure of the hitch! I have pulled my 5er for over 20 miles with the hitch handle unlocked! :S This was a mix of Highway, city and country road including crossing some rail road tracks. The twin jaw will not release unless the handle is pulled.
The location of the jaw pivot points make the the jaws shut tighter the harder the the pull on the pin.
I totally agree it's most always operator error. But I can tell you that the vast majority of damaged beds have a REESE hitch with the handle that sticks up.
Not sure what the problem is but there is some reason so many REESE and Husky and Valley have this problem.
