I have not seen one or heard of one either.
I have heard of only one B&W and it was because the guy had locked the handle open for some reason and forgot to close it.
I totally agree on the operator error theory but there has to be something to make it easier with some hitches.
If you look at a B&W for example that has a 1"thick jaw and your kingpin is sitting on top of the jaws there will be a large gap between plates but with jaws that ate only 1/2" this that gap becomes less visible especially at night.
