I'm going to have to review my hitch setting, I think I have mine set at the middle holes all the way forward. I'll probably end up with the 3" risers like you have. I have the factory air ride like yours.

I am betting you are right. Lowest holes full forward is correct for most applications. Notice there are a straight and angled side to the hitch head supports. To obtain full forward the straight edges need to be closest to the cab.
As far as hitch height 6" clearance is suggested at tailgate.
