I am betting you are right. Lowest holes full forward is correct for most applications. Notice there are a straight and angled side to the hitch head supports. To obtain full forward the straight edges need to be closest to the cab.
As far as hitch height 6" clearance is suggested at tailgate.

Yeah, I was aware of the different ways to place the up-rights from the start but I believe I have the up-rights in the mid holes not the lowest fearing not enough bed-to-trailer clearance. I did intend on having the up-rights as far forward as I could get them, I wanted as much weight forward as I could. :B
I'm going to change that around tomorrow or Tuesday for sure. Thanks for the tip Cummins.
