For Canajuns (from Ontario): GCWR over 11,000kg (24,200lb) you need a Class A license. The only difference between that and a commercial driver's license is that you take the road test in your RV. Also, the rating they give you is AR - the R is Restricted but I've never been able to find out what the restriction is.
Yes, and once over 65 FULL ANNUAL medicals (not questionnaire) - our universal health care is generally wonderful, but it doesn't cover driver's medicals.
If you have a big motorhome with airbrakes, you also need a Z endorsement.
