Are you claiming a trailer hitch has never come off the ball. A ball connection is not 100 percent foolproof:S
Explain how a 5th hitch could come off the Andersen. I've never ever heard of one coming loose. No different than a goose neck ball. Just in a different location.
Apples to apples if you forget to latch any hitch. At least with the Andersen if you forget to latch it then initially the 5er won't fall on the bed. - Road_PhantomExplorer III forgot to chock my wheels. When I dissingaged the hitch, the trailer rolled backwards. My closed tailgate was the only thing that stopped it. Luckily, there was no damage to the rails, but my tailgate had to be fixed. I didn't make a claim on it.

I googled and then punched up "Images".
NO-WAY would i place my 5th on a stupid "Ball".

No its not, but its a lot less likely, then a conventional hitch.
It would be a lot more disastrous with the Andersen if it came off, it wouldn't come off until hitting a bump or something to make it jump off the ball , if thats possible ,if the ball was not locked. - LantleyNomad

If one has the means the Butch device would be a simple one to make a little welding and drilling a couple holes . At one time I was going to build one and get rid of the one I copied the blue ox from. - B_O__PlentyExplorer III don't think Butch's is in business anymore either..Maybe some new form of liability forced them out of business??
