I've spoken with Dave Andersen and the fore and aft adjustments can be accommodated. The optimum placement is relative to truck bed length and position within that bed of the hitch ball. For an 8ft bed on a DRW, its fine to flip both the hitch frame and the king pin adapter. The key is to make sure you still have the 90 degree turn ability without a chance for collision with truck cab to 5er nose. Given the modern designs of some 5ers like Open Range for instance, reversing may also work in some shorter beds.
Flipping just the king pin adapter is a common method to deal with extended king pin boxes.
As for weight distribution, the paramid design properly distributes the weight over the axle.
I've seen various placement combinations in action and never heard of a problem. If you are considering the Andersen Ultimate, give them a call.
