WE had written procedures in power plant from how to operate to how to make safe for repairs.
Everything was written out.
When clearing a piece of equipment for repairs the 'Card' was used every time with check marks at each step. EVERY TIME regardless of how many times one had done it in past.
Nothing wrong with checklist......except when you have one and don't use it!
