I had the same form in my info packet in April of 2016 for our 17 Rockwood. Ours was only $150. The price was specific to brand, model, or size and some Forest River divisions such as Primetime and Coachmen never offered it to begin with. The cutoff date for the 2nd year warranty extension was June 30 2017, I was wrong about the date / year they discontinued it in my prior post. It is not available after that date. There are plenty of posts at the FR forum where people have put up copies of the letters they received from Forest River. I find it hard to believe they still offer it only in select states. Given the level of bad assembly that comes from those factories its no surprise, at least to me, that they discontinued such a program.
