It doesn't sound like you were really paying that close attention to what the clerk was telling you and you got confused with what you were doing. I also suspect that the clerk was going through a "standard" sales pitch to get you to sign up for the credit card and take a survey. Between the two, you got signed up for something you didn't want. Camping World/Comenity should be able to help you remove the credit check and the card from your credit report ON ALL THREE AGENCIES. That is what you should be focused on.
For future and for others, I think it's REALLY important to say that any survey asking you for personal information (driver license, SSN, address, name, exact income rather than range, name of employer rather than type of employer, etc.) should be refused. A survey should never ask for that type of information.
Folks should also know how to dispute something on their credit report. This is done by writing a letter to the credit reporting agencies and detailing the circumstances as to why the item is incorrect - whether it's a late payment that wasn't late, a card that wasn't theirs, or an incorrect address.
