Ok, I tried this and here is the reply I received:
Mr. Lane,
Unfortunately, that is an issue for Good Sam. If you contact Good Sam and see what phone number they have on file for you, that is the number to put into the application. The numbers you have given are not matching up to your Good Sam account, thus, not being accepted in our application.
