I see your fatal mistake- you gave information irrelevant to your location and the operator thought it was significant, therefore mixing it in/up.
Here's what you said:
"We left Needles East bound on I40 headed to exit 9 we are approx. 11 miles East of Needles."
You stuck exit 9 in there when it had nothing to do with your location. Why??? Who knows...nevertheless, because you mentioned it, "exit 9" stuck in the operator's head as significant, and she mixed it in as part of your location.
