That again, would require 2 faults:
Fault 1) H and N are Swapped
Fault 2) An open Ground
Remember we have a "Hot Skin" to EARTH voltage of 50VAC, so Fault 2 is a stand alone fault that anyone can demonstrate by having any 3 wire 120 VAC device that lacks 3rd wire continuity. In all cases there will be a measureable AC voltage from the floating CHASSIS/FRAME/KETTLE to EARTH in a other wise proper installation.
I am not suggesting that we cannot have more then one fault, I am suggesting in all likelihood there is only one fault.
