I have assumed you were plugged into a 30 Amp RV pedestal.
What type of 15/30A adaptor are you using? If it is the "hockey puck" style it is highly likely to be the culprit.
OR
Is a dog bone style? (Normally considered to be OK)
On Edit: I see where the OP has clearly stated it happens with and without an adaptor.
