It very much depends on what campground you're talking about. There's a continuum of campsite size restrictions, and at any level the smaller you are, the more options are available...but you really only need one site that fits your RV at any place, and often that's available.
There are a whole lot of sites that are tent only with walk-in or hike-in access, and even a popup trailer would not work. There are sites that are too small for much other than a pop-up or teeardrop. There are also medium sites, and big sites, and enormous sites, not necessarily all at the same campground. On the whole, though, it's a fairly continuous function; there isn't any magic length where suddenly the number of sites you can use drops off suddenly (excepting perhaps the jump from "I can walk in" to "I need road access").
A 45' motorhome would be quite restrictive in Vermont state parks, which are on the whole among the tightest access in the nation. Many of them were built by the CCC when tents were about all there was. Even my 32' motorhome is on the large side and somewhat limiting, though most of the parks do have at least one or two sites I can fit in and a little bit of flexibility and/or preplanning generally makes it work out fine. An extra foot or two would be similar, but not an extra ten or twelve feet.
