Are you using the proper grade bolt or is it some Chinese replacement of indeterminate strength?
Are you using the proper lock washer/nut?
There was an influx of counterfeit fastening hardware that was exactly like the genuine ones. There is no way of determining the fake ones. This first showed up in the Nuclear Industry, and I'm sure it carried over into all other industries. You pay your dime and hope for the best.
