Cheap Handling Fix - Bolt Missing

This may be an "out there" thought but are you running the bolt inward or outward of the spring? It has me wondering if maybe the bolt is coming into contact with the spring and sheering off? I work on computers all day and couldn't nearly describe the amount of force this would take but when performing the CHF on our RV I did take note that there could be co-mingling of the 2 pieces causing some nastyish results.
