Yes, it should be avoided, and for that matter just isn't necessary. (I'm thinking of the folks that "warm up" their engine for half an hour before leaving a campground, early in the morning and usually in the campsite next to mine ...)
But per Mike Cody at Camp Freightliner, if you kick it up to high idle (I forget what the exact RPM threshold is), it doesn't "count" as "idle." If I recall correctly, this was in terms of Cummins warranty issues, because excessive idling is so bad for your engine that they can hold it against you on a warranty claim.
Or something like that ...
