I was thinking more along the lines of damage to the threads on the lug itself caused by what looks like prolonged contact with the rim. Again noting that it's hard to tell from the pic, if the threads on the lug are damaged a new nut may not stay on for long. Replacing a lug isn't necessarily that hard, but it's harder than just replacing a nut!
I'd recommend that at the very least, those threads be well "cleaned" by use of an appropriately sized die.
