I dropped my Avalanche doing something similar. I didn't have the wheels blocked. It happened so fast I don't know the cause but the jack tipped over and the truck toppled to the right taking the jackstands with it. The jackstands were under the axle and I had the right rear wheel off of it. Jack and jackstands were trapped under it, but thankfully not me. I had a helluva time getting it back in the air. Bent up my jackstands too.
