It is possible that the ground and neutral are reversed.
The ground is the metal tab on the outside of the rig end connector. The neutral should be the small curved slot in the connector. The two with the little dog legs should be the 120 volt.
You should have the same voltage, 120/240, on the connector as in the receptacle.
I sure agree with Doug. Most electricians skip out of the NEC training portion when instructors get there. Most can't read the instructions or diagrams on the equipment they are trying to install. It is very clear in the NEC how to wire the receptacles. NEC Art 551.46 (C) (4)and figure 551.46(C)
