We were asked once at a campground outside Charleston, SC....the Oaks Plantation, I think. There wasn't a problem since ours is well maintained, even though it's 18 years old, but I would have had a real problem if I had reservations only to be turned away after traveling 350 miles from home.
That said, I can understand why the rule exists. Cousin Eddie's are everywhere. It's just nice to know it up front BEFORE you make the trip and get denied. I guess if you're staying at a golf course campground like this one you can get a little more picky:

Never had a problem at a KOA, though.
