In my opinion the majority (I actually believe almost all) blow outs on RV's is low pressure. Low pressure then heat build up then failure. I also believe a large percentage could be reduced with more regular pressure checks as well as the addition of a TPMS. Of course some just can't be prevented with every effort taken.
I have had three DP's over the last 20 years and never had a blow out. Never in an auto either. I check pressure incessantly. In the RV before we roll out every time. One morning I checked and everything looked fine as always. Except it wasn't drivers side inner duel was essentially flat and it didn't "look" any different that the other tires. Had I not put the gauge to it I wouldn't have known. It wasn't a great start to the day but it would have been much worse had I not checked. There are certainly tire issues that can happen while driving even though pressures were spot on before hitting the road. Maybe a TPMS would catch those before an actual failure.
