I have had patients where we sedated to trim the nails back to normal length, these were guys where the nail trim was a challenge for us and the dog, much preferable to do it all at once (for everyone concerned).

My vet said that she's also quicked dogs before under sedation to get the nails to a proper length when the nails are WAY too long, followed by pain meds during the healing process. She said most dogs take this well, with few problems. Good time to get a dental, too.
I trim my dogs' nails weekly; they grow so darn fast!
