Does anyone know what type-Brand Antenna this is?

This antenna had some hail damage on top of it and I am looking for the brand or some more information on this if possible. I am looking to just replace the top part and keep the base still att...

That's a new on to me , also. Depending on how it is attached to the short mast, might be able to replace with a Jack.
