Although using the latest drivers is the correct answer, in your case I would look at older drivers to see if they work.
I know you posted that you tried various OS and machines.
Do you know when and where you were at OS wise and update wise when it last worked.
It is not unheard that updates to an OS has caused problems with things that had worked especially with rarely used software. I am assuming the software is not a common one. By the way, BSD's do leave a trail and maybe someone can debug it?
