As I said in my post it's the constant cycle of discharge & charging that blows the battery. Up down, up down over and over in small amounts will overheat the battery. This happens when you constantly use it while plugged in.

It depends on the specific device. I had a Pantech MHS291L that came with the Millenicom plan that would swell the battery until the back popped off (went through 2 batteries before I upgraded to a NetGear AC791L). I also had an iPhone 5 battery swell until it popped the face off of it. Apple replaced the phone for free even though it was well out of any coverage.
In both cases, I left the devices plugged in all the time (obviously, not when carrying the phone).
