I wondered about that until the first time I used the toilet. When I flushed it, I could look right through the valve and see the ground rushing past. It just dumped on the tracks!
I've never been on a train in America, but can tell you first-hand the trains in Kazakhstan are still set up this way. They just close the toilets when approaching the stations.
