Yeppers. I previously would leave a hose connected and just "turned off" at the "Y" splitter. The most recent purchase happened to be a Harbor Freight one (not because I'm cheap, but because it was convenient- I was there for something else, it was there, you know how it goes). My wife flushed and black water almost flooded the toilet onto the floor (about 1-2" lower than the bowl's rim).
Turns out, the "Y" valve didn't shut off the water entirely. Over the course of hours, the drips/trickle filled the tank up.
Up until recently, I stopped leaving that hose connected. Now, I leave it connected but have 2 shut offs- one at either end of the hose. Both would have to fail for me to have a repeat.

And ditto on the rupturing of a tank. Happened to a buddy of mine when he spaced out and turned off the hose
twice.
