They are both choices. Which lives are most important?
only those actually involved in the accident are hurt. Bystanders that witnessed the accident are not impacted.
However you may spread the virus to someone you didn't even know. Someone far from the source may die that had no contact with the original infected person. The infected person may totally survive the accident and have no systems , yet he may spread the virus to lots of unsuspecting people which may have devastating results.
