I used to work for a pain management doc...a good one can give you a new life. Strongly suggest you contact Ohio State Univ Med Cntr to see if they have this specialty there. Or Cleveland Clinic. Our DS is 50 years old and was evaluated at Vanderbilt Pain Mgt...it has improved his life considerably and his PCP manages the meds based on Vanderbilt recommendations so no long drive every 30 days. Start at the top...there is help out there. Please don't give up without giving this a try.
