Being in software development, you'd be amazed at some of the simplest things that get missed. People are looking for functionality and not necessarily content.
Way back, I once watched a flyer go through 7 different people for approval. Everyone reviewed it and "stamped" their approval on it. It went to print. It had the wrong date on it.
Sometimes it's the most obvious things that get missed!
