There is a little more to this thread then I care to read, so hopefully I'm not repeating. But...
Have you simple taken the thermostat off the wall and disconnected one of the wires? If that stops the fan after a few minutes then the thermostat either can not go low enough or it's defective.
If that doesn't stop the problem then it might be your furnace time delay relay.
