I carry a list of my meds (15 of them) on a printed sheet that is not much bigger that a business card.
The list is copied several times and when I go to one of my docs I just give the list to the nurse and she puts it on their fact sheet. Saves a lot of time.
You can do the same if you have Microsoft Word as it is easy to make it to the size you want.
