I use the exhaust brake in Auto mode when empty and tow/haul off. If I get into traffic, tow/haul comes on to give me better shift points. Once I add my 4000 lb camper to the back, I switch to exhaust brake with tow/haul stilll off unless in traffic. When I have my camper and 8000 lb trailer, exhaust brake goes on full and tow/haul comes on. The settings typically depend on load, terrain, traffic conditions and driving style. The only time I drive with both off is when conditions are slick and I want soft shifts up and down without exhaust braking.
