coupler is correct when setting up a Blue Ox Sway Pro. If that requires the head shank to be flipped then so be it, if that creates ground clearance issues with your particular TV you may be forced to invest in a custom shank. I'd agree with an earlier poster ... since you have so many questions you'd be better off to contact Blue Ox directly rather than rely on WAGs from some who don't own or have probably never even seen a Sway Pro. ;)
